Make check payable & Return Forms to Brentwood Baptist Deaf Church

Be sure to have medical forms notarized. Forms can be mailed in or brought by the offices of Brentwood Baptist Deaf Church.

Drop Off / Pick up Locations (These Locations are subject to change)

Option #1: First Baptist Church Nashville

Meet in the back parking lot corner of Broadway and 7th Ave. next to Gaylord Entertainment Center (Arena)

Option #2: Hardees Antioch

Corner of Harding Place and Nolensville Pike 4099 Nolensville Pike

Dropt off time: 8:15 to 8:30 a.m. (white bus will leave at 8:30 a.m.)

Pick up time: 5:00 p.m.

Option #2 Brentwood Baptist Deaf Church

Door 1 (Inman Deaf Chapel Door)

Drop off time: 8:45 to 9:00 a.m.

Pick up time: 4:30 to 5:00 p.m.

Late Fee Charge: $1.00 per minute per student for pick up after 5:00 p.m.
